A Case of Acute Intermittent Porphyria Resembling Conduct Disorder with Psychotic Features


Abstract
This is a brief report on a case of acute intermittent porphyria observed in the Gyeongsang National University Hospital.
13-year-old korean boy was admitted to the department of psychiatry due to lies, running away from home and violent behavior, During admission, however, he showed abdominal pain, agitated behavior, persecutory delusion, auditory hallucination and
emotional instability which could not be controlled by conservative medical treatment. Consequently further observation, family study, and various laboratory tests were done.
The diagnosis of porphyria was established by Watson-Schwartz test and amount of delta-aminolevulinic acid and coproporphyria in the 24 hour urine collection, He was received high carbohydrate diet and 200mg/day dose of chlorpromazine, and showed
good
response to the treatment given.
From this case experience and literature review, authors stressed the importance of 1) interdepartment consultation, 2) the needs of careful review and evaluation of the results of the basic and further laboratory tests for which psychiatrist
tend
to
pay less attention to those cases with psychotic manifestations.
